Duplex scan and copy prints a stripe of content offset to left

Product Name: VersaLink B405 Multifunction Printer
Operating System: Windows 11

When I scan or copy duplex using the adf, the first page is fine, but a 2cm(ish) strip down the middle of the duplex page is printed offset to the left, overprinting what should be there (?!). So everything gets read, everything gets printed, it is just a stripe from the duplex page get printed in the wrong place leaving a thick black stripe where it should be. I am also getting a 065-225 error...

Struggling to see how this can be mechanical... I then updated the firmware and that has not affected things.
