Is this from a different machine than the one the screenshot is from? Why is it going to a folder called "h1"?
I think you may need to get Xerox 2nd level to analyze a wireshark trace of it working and not working to figure out what is happening.

.10 being the Workcentre client and .11 being the FTP server.
What does the eventlog on the ftp server say when the scan fails?
Below is a picture from our address book, showing exactly what is going on. This still only allows stores to send 1 PDF per day using the same name, where it should allow multiple times a day and affix a 000 and 001 at the end of each file. Thanks for your reply.
Sounds like somebody changed the filing policy on your devices.
In CWIS go to Address Book and select one of the FTP addresses and choose Edit
Then check that the Filing Policy is set to Change Name and that the Folder options are as seen here as well.
If this is not the correct answer to your issue, please provide the full details of how the scan is setup and I will see what I can figure out.
Recently been running into issues with our Xerox WorkCentre 3325's. FTP is working correctly to onsite server from multiple stores, but stores are only able to send 1 file (PDF) into the server. In the past we experienced multiple files sent, and would affix 000 onto each file after the first file. For example:
PDFFILE
PDFFILE(000)
PDFFILE(001)
There should be no reason a store wouldn't be able to FTP multiple files a day.
After this, multiple attempts fail until the subfolder is deleted. Once deleted, the printer recreates the subfolder and can create the PDF file again. Very frustrating.
Anyone have any ideas, or have had issues like this in the past? We have tried different FTP servers and have experienced the same result.
