I'm having the same exact issue however I'm using an internal open SMTP relay.
Scan to Email and Scan to Folder work fine as seperate functions; it's just the fax forwarding that does not work.
I do not receive any error messages and I've upgraded to the latest firmware.

I've a similar problem with ColorQube 9303. The controller of 9303 and 7545 is the same so I think it colud be the same problem I have.
If you want the colorqube to forward your incoming faxes to an email using smtp authentication it delets the "to address" field so it fails to forward. I managed to understand this reading the details on job history and comparing with the confirmation report (in the confirmatin report the address was specified while the job details reported that no address where specified).
Xerox II level told me that it's a Known issue...but still no solution and they must deploy a new firmware.
The fax forward on the 7545 was solved? The firmware was released? I don't see any other replys.
Years later and I'm having what seems to be the same issue... was running acient firmware so I expected update would resolve but no joy.... does anyone have advice?

SMTP works for scan to email... Embedded shows 'enable', followed the manual...
"Job was not sent to the SMTP server" for fax foreward..
