Problem solved!
Steps I've taken to be able to scan to SMB folder again:
1. Install Windows 10 (good bye my fav win7) and enable SMB 1.0
2. Temporary disable antivirus and firewall
3. Do not add a "Document Path" when adding a contact (unless there is a subfolder)
4. Change pin to password for Windows login (hint if selected Picture login, Pin login or Fingerprint login, SMB scanning do not work)
5. Change Dual IP only to IPv4 on printer
6. Configure DNS settings on printer web page
7. Change default port to 445 when adding a contact
I think steps 1-4 were the most important. It was a journey for sure. Don't think a typical user (hi, mom) would ever be able to figure this out.
Side note: "Unexpected communications error" when trying to discover SMB folder from printer's interface never got fixed. My hands are itching to figure out why but I guess some things should be left alone. Especially with Xerox.

I have a few things to add that I hope you find helpful.
If any of the properties of your network connection changed then Windows most likely defaulted it to Public network settings when you connected to it and most of the relevant services are either turned off, or firewall blocked, by default on Public networks.
After 2 days of trial and research I realized that SMB might indeed be an issue. On my Win 7 I do not have "SMB" as a Windows feature. Apparently, it's normal for Windows 7's. I tried adding SMB function via PowerShell and Reistry but don't think it changed anything. After resetting the printer now I am getting an error code 016-426. I double checked IP, DNS and other settings and everything seem to comply with the instructions. I still can't Browse SMB to add new contact to perform Scan To. :(
Check to make sure that SMB 1.0 is turned on. Open Control Panel and select Programs and Features and then Turn windows features on or off. Go down the list and make sure SMB 1.0/CIFS file sharing is checked. Restart using Start > Power > Restart
I am an individual trying to setup my Workcentre 6515 to Scan. I used to have no issues with Scan To - Contacts - SMB file. However, after factory resetting my printer when I try to add new contact SMB I get this error:
"Unexpected Communication Error. Unable to Browse SMB. Connectivity issue."
I also couldn't scan into the contact folder I used to scan before, error message 078-755
I spoke to support yesterday and we checked my IP, DNS, WiFi, Ethernet and folder share setup. Everything is setup correctly but the issue is still on. I even deleted my antivirus thinking that firewall is the reason, but it's not.
Please help. I won't be able to be on hold for over 2 hours again :(
