WorkCentre 5735 invalid home path contact sa

Product Name: Other - specify product in post

5735 WorkCenter has LDAP configured.  I am able to go to User Mappings and successfully do a test.  When sending an email, I can seach the address book successfully.  I have setup a File Repository, to point to the server \\120fs1\network, and can successfully do workflow scanning to that folder.  In the LDAP settings, under custom filters, we do have it set to Append base DN as domain\USERID.  This lets us login to the device as just username, as opposed to username@domain.com or domain\username.

Scan to Home is setup to do LDAP Query, scan to a subdirectory, and Login credentials are et to "Authenticated user and Domain".  I am able to successfully login to the WorkCenter with a domain account.  But when touching ScantoHome, on the right, it says "Invalid Home Path, Contact SA".  I'm not sure what to.
