Xerox 550 - Can't use pre-printed cover with InDesign on Mac OS X - 10.6.8 or 10.5.8

Product Name: Other - specify product in post

I have a Xerox 550 and I am using Adobe InDesign CS5 to try and print a booklet with a pre-printed cover in the inserter tray 8.

I can print a regular booklet without any issues.  But when I try to use a pre-printed cover, the output is messed up.  It will either print two-up on 1/2 of the 11x17 paper where both pages are on half of the 11x17 paper (so, two pages are on one side of the 11x17 paper).  Or, it will print the last page of the booklet as the first page, print half of the pages in reverse order, and skip every other page so that only two pages of the four are printed.

I was able to export the booklet as a high-res PDF, use the pre-printed cover and print from the separate (Mac) Preview application, but the image clarity is poor.  I even exported the PDF without compression, and the image quality still stinks.

Has anyone had any experience with printing a booklet and using a pre-printed cover on a Mac with InDesign?  Is this even possible?

Thanks.
