4620 Paper Tray is Lost

Product Name: Other - specify product in post

We have 3 trays on our 4620 printer and as of last week, only 2 are showing up. Tray 1 - Bypass, Tray 2, Tray 3 is no longer a valid option. I tried going to the IP address but have an issue as it asks for the Username and Password and the default does not work. I just want Paper Tray 3 back. Can somoene help me?
