OK, so it is printing what is termed a "Menu Map"
If it was printing a startup page with it, it would imply a dead CMOS battery as it would just mean all the settings are lost at reboot so it prints like it was factory fresh (The menu map only prints at first boot automatically).
But since you are not stating that the IP and defaults are not changed, and that the start up sheet itself does not print either it appears to eliminate that idea.
That leaves corruption. Which would mean trying an NVRAM reset (This will wipe out every setting on the device back to factory defaults)
Before moving forward I would print a configuration sheet so you have a record of any settings you would need after the reset.
To NVRAM reset, At the control panel select Troubleshooting>OK>Service Tools>OK>Reset NVRAM>OK>Exit
Then reboot and see if it is still happening, if it is, a service tech would probably be required
Thank you for the reply, however, the Startup page has been disabled. The page that prints is not the Start up page. The page that prints has descriptions of the various buttons on the control panel and what they do.
