077-120 paper jam on same page of every set

Please take a look at both the cases about the fault code 077-120, here is the first one if you are using hole punched paper if that is not the issue look at the second case about the code.  If these do not help please consider contacting your support centre for further assistance.

I'm getting this fault code on almost every set.

fault code 077-120 paper jam

specs:

DC250 machine

11x17 sheet size

24# multipurpose white

black and white duplex

62 sheets per set

feeding out of overside document feeder

What's weird is that it's the same page, and the sheets stop in the machine in the exact same position

any ideas?
