B315 keeps printing out a sheet that is not a banner/confirmation page with every print job

it's printing from mainly adobe reader and Google Chrome, we uses Google sheet, google docs, etc. 

it's still giving out this sheet and we haven't been able to narrow it down to where

What application is the print job originating from?

Google search appears to indicate that it may be a setting within the application where print Document Properties/Summary is selected.

Anybody else have any other suggestions on what this page that got printed out may be and how to turn it off?

Hello Brew,

All I can find are the steps to turn off the banner page from 3 different locations. Maybe review those to make sure you didn't miss it in one of the 3 areas.

Need help to stop this page from printing with every print job on the Xerox B315 

From what I can tell after scouring thru the settings on the printer itself and print preference, this is not a confirmation page, transmission page, nor a Banner page, already turned those off.  This is wasting a lot of paper and toner un-necessarily
