Cannot reset B215 printer to factory defaults.

Product Name: Xerox B215 Multifunction Printer
Operating System: Windows 10 x64

I want to reset my B215 printer to factory defaults.  I have uninstalled all B215 software & accessed the printer via its IP address.  It wants a user name and password to do this.  I don't think I ever set a user name/password.  I have tried:

  User names   admin, Admin, 1111, and the MachineName

  Passwords   1111, SerialNumber

I have tried unplugging the B215 for over 10 minutes,

Nothing seems to work.  Any help will be very much appreciated.
