Custom page text position measured from top incorrect

I eventually managed to hack a solution together by setting the page length to be 8mm larger than the actual page length but keeping the document size the same.

When set like this the driver positions the document centrally on the page thus moving it down on the actual, physical page.

It still doesn't seem correct to have to do this but it works.

I'm attempting to print a custom sized page from the bypass tray, however the position of what's printed seems to be shifted upwards by about 7mm. The horizontal position is fine as is the overall size (as it's printed as actual size from Adobe Reader).

The document was originally created in InDesign, is 76mm x 204mm and has no margins. I need the text to be printed in a precise place on the page, with the first text item positioned about 8mm from the top and 18mm from the left. However, when physically printed the text appears right on the top edge of the page but the left positioning is okay. 

I've set up a custom page size of 76x204mm in the printer and done the same with the driver. Adobe Readers preview looks correct.

The pdf's I'm printing have worked fine for years using my old HP printers and the same custom page size.

I'm using Xerox Print Experience 5.1, App Version 7.211.24.0, Driver Version 7.158.0.0, PCL6
