If you are seeing this using just normal 20lb weight paper, then the fuser probably needs to be replaced.
If the paper is heavier than normal 20lb paper then read below.
This is usually caused by an incorrect paper weight (gsm) setting when loading the paper in the tray. Make sure that you set the proper paper weight (gsm) when installing this paper in the printer (set to something like heavyweight or extra-heavyweight if unable to set the specific gsm). Thicker paper needs different fuser settings (heat and pressure (time)) in order to allow the ink to set into the paper correctly. Otherwise the ink will smear (offset) like this.
Note: You can clean the excess toner off the fuser rolls by running about 20 BLANK sheets thru the machine prior to attempting the print job using correct paper settings. Do this until the blank pages come out clean.

Hi,
Take out the fuser module and check its surface. It could be damaged. Also be sure to select the correct settings for paper.

Hi,
I have an error with my Xerox Phaser 6500. I received a new black cartridge today to print but the black color seems to tear apart. I printed multiple times and tested it with an OEM Cartridge but the same result seems to happen across both cartridges. I also cleaned the Drum Unit because before this it was printing just all faded gray.
Any way to fix this, please? I hope it's fixable and it doesn't ruin the printer. Any help would be genuinely appreciated.
