How do you turn off the cover sheet when printing on the Workcentre 3345

When I print from my computer, print looks great, but, there is one thing very annoying. It prints out a cover sheet that tells the file name. How do I turn this off.

Start > Control Panel > Devices and Printers

Right-click the 3345 and choose Printing Preferences

Click on the green square (1)

click on No Banner Page (2)

OK(3) > Apply(4) > OK(5)
