I use a Xerox Phaser 6600 to print custom size envelopes, and i had a workflow set up whereby i regularly printed the envelopes from the InDesign files. I use the bypass tray on the printer, and had very precise alignments and envelope sizes. The setup worked flawlessly for years until the printer itself broke. I recently replaced the printer with the exact same model, which necessitated installing a new driver for the printer. Today when i attempted to replicate the workflow with the new driver in Indesign, the printouts come out the size of a postage stamp on the upper right corner on the envelope, whereas before, i obviously had a full sized print fully in line with the size of the envelope. I'm really confused – i've set the document size in InDesign and let InDesign control the document size with the printer – i've checked if any settings are different from the previous printer driver, and can't find a single one – and yet what i get is a postage size stamp mini printout. I thought it might be an issue to InDesign specifically, but when I exported a PDF file of the document and printed it out using OS X preview and i got the same issue. Have i missed something obvious? Enclosed are screenshots of the printing settings, the document size settings, and the OS X custom page size i used in preview.

Would really appreciate anyone's insight to this problem. My business relies on this envelope workflow so i'm kind of in a tight spot until i debug this issue. I'm using the latest version of InDesign and OS X – and the latest version of the printer driver that Xerox provides. The only issue I can think of that's different between the newer printer and the old one that just broke is that the older one was using an older version of the printer driver.

I apologize, I have been trying to find a 6600 on the LAN to run some tests and have failed miserably for an hour.
I have some feelers out to colleagues to find one for me to test on, I will update here if I can find one, with my findings.
It happens regardless of that setting in Acrobat Reader and it also happens when printing from Preview or Inkscape. If I keep adjusting the custom width upwards it starts to properly respond to the custom size around the 200mm width.
I am running El Capitan and the downloadable Xerox 3.64.0 driver.
That happens if you select the "Choose paper source by pdf page size" in Acrobat/Reader, I have never seen it happen without that variable enabled.
Is this with the Apple supplied Generic PS ppd or the Apple supplied version 3.51.0 6600 driver, or the Xerox.com downloadable 3.64.0 (1572) driver ?
And what OSX version do you have running?

Everything looks fine in the preview on the Mac side, but the printer prints the document in a little tiny square in one corner of the customer page. I have tried printing from different programs, but the results are always the same. When printing one of the selectable standard sizes, the printer prints fine.
