I have the same problem on my 550 with integrated Fiery. Printing to 12" x 18" stock and trying to do manual duplex, there is an image shift of almost 5mm from the left edge to the right edge. Tray alignment does not resolve the issue for me either. We print a lot of business cards (double sided) which means that we have a major problem if we can't fix this. Please help me, too.
I've gone through letter, tabloid, tabloid extra and 13 x 19 tray alignment and this has no affect on the fact that Photoshop (CS3 or CS5) will not print centered on the page. Illustrator CS3 and CS5 will print the same file perfectly centered on the page. This seems like a photoshop issue, not a Xerox issue. Equipment is the Color 550 with bustled Fiery. Specificallly, the margins are offcenter by2-1/2 16s of an inch, always pushed up and to the left.
Depending on which Fiery Controller and Xerox printer you have, there is a feature called Tray Alignment (Align Tray). You can run through this feature to allow you to adjust the alignment of the image on the sheet of paper. You can find this feature in Command Workstation.
It is a bustled fiery. I am using xerox color elite silk paper. I do not think it is a paper issue but is a fiery calibration issue. No matter what I try, it is off center about 3/32 of an inch to the right.

What kind of printer do you have?
Does it have a RIP server attached to is (Freeflow/Fiery/Creo/Splash etc)?
When I print on 12x18 paper on a single side, either from a pdf or directly from InDesign, the paper will not center the image. It prints one margin larger than the other. How do I get it to center vertically and horizontally? When I run a 11x17 booklet out of the bypass tray, it works fine, even with duplexing.
