I don't have a Mac to screenshot or troubleshoot with, and certainly can't get access to 10.11, but lookin the driver for Job Type and be sure it is set to normal and not sample print.

Prints ad infinitum

Product Name: Phaser 3040
Operating System: Mac OS X 10.11 - El Capitan

I have a Xerox Phaser 3040 which I am using to print 3x5 index cards. Whenever I print a card, the printer prints one card and then sits there with the green ready light flashing. If I then press the start button, the printer reprints the same card again. Each time I press the start button, another copy of the same card is reprinted. (Yes, when I print, copies is set to 1 in the print dialogue box.) This is true even if I have queued up another card to be printed. The same card will be reprinted ad infinitum until I either press and hold the start button to cancel the job or cycle the A/C power off and back on. I have this issue using MS Word, Pages from iWork, and TextEdit. All behave the same.
