I cannot find anything in the knowledgebase that describes what you are showing in that picture. Does it happen on all applications? For all users?
The only thing close in the knowledgebase is the watermark feature. You can take a look to see if that is turned on.
If that doesn't help, I would recommend contacting Support at 1-800-821-2797.

It looks like you need to disable "Print Settings On Page" in the Image Options section of the printer preferences. If you want to disable it permanently open the control panel, right click on the printer, select Printing preferences, click on Image Options in the window that opens, uncheck Print Settings on Page, click OK.
That settings is in the driver primarily for setup troubleshooting. I use it when I'm trying to fix color rendering issues and I want each of my printed samples to show me what I changed. Once I settle on the settings that I want I turn it back off.
