Just tried and it worked fine from windows 7 x86 and Windows 10 x64 with the latest driver from EFI here.
The issue you are seeing does happen if you have the wrong driver (1.0 on a 1.2) I have heard. Also, don't use the driver from Xerox.com when you have an EFI rip, that tends to be an issue.
There are occasional issues where page numbers become a problem with EFI, and since you mention duplex, I will quickly sum that issue up.
Say you have a 10 page job and you set a slip-sheet after page 6.
Simplex should be printed 1,2,3,4,5,6,slipsheet,7,8,9,10
Duplex should be 1-2,3-4,5-6,slipsheet,7-8,9-10 when thinking of the pages digitally. But..
I have seen drivers from EFI consider it in physical sheets instead of digital pages. So the 10 page job is actual 5 physical sheets duplexed, and in that case the print job never makes it to the "page 6" to add a slip sheet, so it just doesn't add it. Usually a driver update will fix this.

Our Versant 80 only recently installed has a problem. When a job is duplexed the slip sheet feature doesn't work.
The Xerox techs have so far been unable to fix the issue.
Does anyone know if this is the same on all Versant 80's.
We are running CWS latest version on Windows 7 with a Fiery EX-i80 RIP.
