I have Xerox Versalink B405. It was working fine but suddently it stopped working and I am getting following error on screen
Device fault
1. Power off the device
2. Wait for all control panel lights to go out
3. Power on the device
If the problem persists, refer to online knowledge base, or contact you system administrator
Fault or Error 116-313
I following the above procedure but device is getting stuck on above error message.

Device is stuck on the error screen. it is not moving ahead. I cannot access device web interface. i cannot ping the device IP address.

In this condition you need a tehnician to do this, because is a part of diagnose mode.
For upgrading de firmware do the following;
Download the firmware (.bin) file, refer to Obtaining Machine Software.
Create a folder named ‘DWLD’ on the USB memory device.
Store the software download file (.bin) in the ‘DWLD’ folder
Select Download Mode on SPECIAL BOOT MENU 1/3, then touch YES. The UI will display SW update progress. The machine will eventually reboot
I had the same issue. Followed the above instructions. During Upload i got the following:
INITIALIZATION ERROR 016-741
Cannot find it listed in the Community. Next steps?
You need a tech to restore to 0 one NVM value and to reload software.
