Try this:
Log in as "admin", go to Tools>General and scroll down to Erase Customer Data.

Thanks CathyO, but that didn't fix it. Still prompting for UserID and Account ID.
Is there a way to completely factory reset these things? I thought I had, but these settings remained.

I searched the Knowledge-base and found the following solution to turn off accounting and authentication from CWIS.
WC7830
I turned on Accounting as a test. Now I can't get rid of it. I've set Properties -> Login/Permissions/Accounting -> Accounting Method to None. I still get a prompt for User ID: and Account ID: on every printout.
What am I missing?
