I ignored the problem for about a year, as I was able to use an older mac for the limited amount of printing that I typically perform. Finally, with the arrival of other Mac users in my office, we needed to resolve the issue.
A call to Xerox technical support quickly identified the need for updating the Xerox system software.
Our Xerox was running 061.121.222.06502.
All we had to do was install new software via the zip file WorkCentre_7500-system-sw06112122336000.zip, downloaded from https://www.xrxgsn.com/admin/public/3452/WC75xxSpar.html .

Did you find anything out with support? I'm curious to see that actions they had you do to resolve the issue. I'm guessing clean the drivers out and then re-install - or a SPAR driver?

Thank you for using the Support Forum. Have you configured the new driver for accounting?
Open Safari web Browser type http://Localhost:631 in address bar > Select administration> Manage Printers>Select link for Xerox printer
Select drop down for Administration>Select Default Options>Advanced> set accounting system to Xerox Standard accounting.
Set to always prompt and Select Set as default button.
If you have already tried these steps then please consider contacting your support centre for further assistance.
Printing with accounting enabled was working well when using OSX Snow Leopard and Lion. Having upgraded to Mountain Lion, I now get invalid accounting id print outs. My other Mac (running Snow Leopard) continues to print successfully.
Note that I have already tried recreating the print queue after installing the latest driver.
